Management and outcomes in pulmonary arterial hypertension patients with sepsis

A recent retrospective study examining sepsis treatment in patients with and without pulmonary arterial hypertension (PAH) reveals critical insights for clinical practice. Among 30 sepsis patients with pre-existing PAH and 96 matched controls, those with PAH received significantly less fluid resuscitation but more vasoactive medications. Notably, PAH patients who received less fluids had higher mortality and those who received no fluids had a longer time to receiving antibiotics, indicating a potential delay in recognizing sepsis. PAH patients who were given more fluids experienced lower mortality rates and shorter times to antibiotic administration, highlighting the need for timely recognition and dynamic fluid management in this vulnerable population.
